Margate Health and Rehab is hiring for a full-time Admissions Coordinator
General Description : Admissions Coordinator assists Admissions Director with the admission process.
Duties and Responsibilities :
Responsible for census development. (In facilities where there is a marketing director, this responsibility may be shared)
Daily ongoing contact with all referral sources (hospitals, local Department of Social Services, State Bed Registry, home health agencies, hospice, local Senior Centers, etc.).
Provide information and tours of facility to prospective resident / family members.
Promote facility in community (public speaking, participation in local groups, etc.).
Manage Providerlink or other referral sources.
Prepares ongoing reports that track census development.
Responsible for beginning admission process.
Receive, review and route FL-2’s to appropriate department heads.
Maintain regular ongoing contact with all referral sources regarding status of prospective residents.
Make final arrangements for admission (alerting department heads to resident admission, making transportation arrangements for resident, etc.).
Responsible for initial admission interview with resident and / or responsible person.
Review admission packet with resident and / or responsible person.
Answer all questions regarding facility and admission process.
Obtain signatures on all necessary paperwork and distribute paperwork to appropriate places.
Be available to assist with other duties as needed.
Working Conditions :
Works in office areas as well as throughout the facility and moved intermittently during working hours.
Is subject to frequent interruptions.
Hours are flexible. May need to work beyond normal working hours, on weekends and holidays when necessary.
May be subject to infectious diseases, air contaminants, and / or hazardous
chemicals.
Qualifications :
Must be able to read, write, speak and understand the English language.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies / personnel, and the general public.
