Job Summary: This position is responsible for providing case management and outreach services which promote quality, cost-effective outcomes by helping high risk/complex patient population achieve effective utilization of healthcare services. Case manager will collaborate with the health care team to assess, advocate, implement, coordinate, monitor and evaluate healthcare options and services so that the team meets the needs of the individual patient. Case manager will assess these patients for social determinants of health that may be causing barriers to their health improvement, assist the patients to set self-management plans and goals, will provide resources for patients and family members to meet those goals, and will coach/encourage patients by follow-up contact. Functions as a member of the healthcare team within patient-centered model of care to enhance the patient’s quality of life.
Essential Functions:
• Assists with the coordination of care for patients as directed by the healthcare providers
• Completes an assessment of patient/family needs with respect to physical, psychosocial, educational and documents all other needs and acts as a resource and coach
• Assess the patient’s formal and informal support systems
• Act as a patient advocate by coordinating necessary resources and working with interdisciplinary team to achieve goals, objectives and integration of care
• Makes recommendations and assists patients entering mental health and substance abuse programs
• Assures that follow-up clinic or specialist appointments are made, to get the assessments or screenings completed as directed by care team
• Outreach and develop working relationships with other agencies whose services will be beneficial to patients
• Other duties as assigned by supervisor
Competencies:
• Good Proficiency and navigation PC application included by not limited to MicroSoft suite of products and EMR scheduling systems
• Ability to multitask while actively listening
• Excellent verbal, written and keyboarding skills
• Good telephone manner with a clear speaking voice
• Effective communication skills and the ability to build rapport with the patients and outside professionals
• Ability to work well within a team atmosphere
Qualifications
Required Education/Experience:
• Bachelor’s Degree in Social Work
Preferred Education/ Experience:
• 1 years of experience in Case Management
• Bilingual in Spanish
