What are the responsibilities and job description for the CENTRAL REFERRAL COORDINATOR position at Mariner Healthcare?
SUMMARY
Supports the Admissions Department activities in the pre-admission and admission process.
ESSENTIAL DUTIES AND RESPONSIBILIIES include the following (Other duties maybe assigned):
· Reviews and approves resident/patient referrals.
· Monitors the inquiries for admissions and follow-up in a timely manner (within 10 minutes or less).
· Tracks pre-admission referral activity daily/weekly. Keeps a daily log for weekly reporting.
· Coordinates the admission, makes arrangements and confirmation of all needed admission information. Notifies Departments of anticipated admission for the assigned buildings. Submits all the required documents to concerned building with the admission.
· Verifies insurance coverage/eligibility and runs the sex offender’s website on all patient referrals for potential admission and secure admission authorizations prior to admission.
· Ensures proper identification and coordination of specialized equipment/DME prior to admission, including other necessary clinical information to insure a smooth patient transition upon admission.
· Other assigned duties and responsibilities.
QUALIFICATIONS
· Bachelor Degree preferred and or with LVN license.
· One or more years’ experience in healthcare /insurance or commensurate education preferred.
· Proficient in the use of personal computer.
