Job Summary
We are seeking a dedicated and experienced Case Management Supervisor to join our team. The Case Management Supervisor will be responsible for overseeing a team of case managers in the field of aging or disabilities. The ideal candidate will possess substantial experience in case management and demonstrate strong leadership skills, with a focus on improving outcomes for clients in the aging or disabilities sectors.
Key Responsibilities
- Supervise a team of case managers, ensuring compliance with all organizational and regulatory guidelines.
- Provide ongoing training, guidance, and support to case managers to enhance performance and client satisfaction.
- Conduct performance evaluations and facilitate professional development opportunities for staff.
- Review case management practices to ensure the appropriate allocation of resources and timely service delivery.
- Address and resolve any issues or concerns related to case management and ensure high-quality service provision.
- Maintain knowledge of relevant laws, regulations, and best practices in the fields of aging or disabilities.
Qualifications
- Educational Requirements:
- A bachelor's degree in a health or human services field from an accredited college or university.
- At least one (1) year of experience in a health or human services field, OR the educational/experiential equivalent in the field of aging or disabilities.
- OR, if a registered nurse:
- At least two (2) years of experience as a professional nurse in the field of aging or disabilities;
- A master's degree in a health or human services field from an accredited college or university.
Experience Requirements:
- Must have at least four (4) years of experience as a case manager in the field of aging or disabilities.
Skills and Abilities:
- Strong leadership and supervisory skills with the ability to manage and motivate a team.
- Exceptional communication and interpersonal skills to work with clients, staff, and external.
