Marquis Home Care -
Pay Rate : $150-$200 per visit
PHYSICAL THERAPIST : Per Diem to cover capital region for agency admission on weekdays and / or weekends

What you will do :
Provide physician-directed physical therapy services by, initially and periodically evaluating patients’ needs, establishing a goal-directed plan of care, and initiating planned care. The plan of care and re-evaluation occurs no less often than every 60 days.
Provides quality care within guidelines of department policy and procedures as well as local, state, and federal rules and regulations and the Code of Ethics as established by the American Physical Therapy Association.
Provides written documentation of assessment and care delivery in a professional manner according to policy and within the designated period.
Ideal Candidate :
Active NYS Physical Therapy licensure
One year of physical therapy experience is preferred, however, new graduates are welcome to apply
Previous homecare experience and / or acute care or rehab experience is a plus
