What are the responsibilities and job description for the CHAP CASE MANAGER position at Marrakech, Inc.?
Marrakech is looking for staff to work with our wonderful participants with developmental disabilities, physical, behavioral or other needs in a group home setting. We seek nurturing people who will provide care with respect and dignity as well as promote independence and community integration.
OVERVIEW
The CHAP / CHEER Case Manager will work with the participant to find and set up housing, increase life skills and financial literacy, and meet personal education and employment goals.
DUTIES
Responsible to ensure that the youth learn the skills to cook, shop, budget, self-care, travel, security, and self-sufficiency.
- Check-in with the young adult to ensure they retain school grades for CHAP or gain or keep employment for CHEER.
- Teach them their community including traveling to medical appointments, local businesses such as grocery stores, laundry facilities, banks, etc. Able to work a flexible schedule, which may include occasional evenings and weekends. Person of interest MUST be DCF credential or have the ability to get credential prior to start date.
EDUCATION and / or EXPERIENCE
Bachelors degree (B. A.) in Social Work, Human Services, Special Education or related field is preferred.
A high school diploma and seven years of participant development, social work, family supports, or case management experience will be accepted in lieu of degree.
