Position Information

- Reports to and works closely with the Coordinator of Case-Management and Student Development and other administrative staff.
- Works closely with the Director, Assistant Director, and Coordinators to ensure quality care is being provided to clients.
- Participates in the on-call schedule and managing crisis situations.
- Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.
- Counseling Center outreach events
- Tracking events and goal progress for the Counseling Center
- Scheduling meetings and sending out Teams links weekly
- Works closely with the Mental Health Specialists:
- Carry caseload for med management and higher acuity clients for more hands on services
- Walk-in clinic and intakes
- Assessment
- Developing, monitoring, and evaluating treatment plans and progress
- Communicate client needs to a variety of care team members and follow up accordingly and provide appropriate referrals
- Linkage and referring students to community resources
- Maintain client records by reviewing case notes and monitoring files
- Advocate for client’s success
- Scheduling/rescheduling psychiatric appointments
- Psychiatric medication refills
- Weekly psychiatric provider reports and weekly communication with medical staff
- Monitor psychiatric appointments and communicate any concerns with Mental Health Specialists and medical staff
- Document and monitor student attendance, outreach to clients who are missing appointments to ensure continuity of care
- Manage discharge plans upon completion of treatment
- Assist in development and carrying out workshops and support groups
