Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting Previously Homeless individuals providing guidance, resources, and advocacy. This position requires a strong understanding of workforce development, behavioral health, and social services to effectively assist clients in navigating their challenges and achieving their goals.
Duties
- Conduct comprehensive assessments to identify client needs and develop tailored service plans.
- Provide crisis intervention and management to clients facing immediate challenges.
- Facilitate access to supportive housing, healthcare services (including Medicare), recovery, treatment, employment, and leisure.
- Offer mentoring and supervision to clients, empowering them through skill-building activities.
- Collaborate with community organizations and service providers to enhance client support networks.
- Maintain accurate documentation of client interactions, progress, and outcomes in compliance with organizational policies.
- Monitor client progress and adjust service plans to ensure effective support.
- Advocate for clients' rights and needs within various systems, including social work frameworks.
Experience
- Proven experience in case management or related fields, such as social work or behavioral health, is preferred.
- Familiarity with workforce development programs and crisis intervention techniques is highly desirable.
- Experience working with diverse populations, including children and families, is a plus.
- Strong interpersonal skills with the ability to build rapport with clients from various backgrounds.
- Excellent organizational skills and attention to detail for managing multiple cases simultaneously.
- Ability to work collaboratively within a team environment while also demonstrating independence in managing caseloads.
