Salary Range: $110,000 – $140,000

We are actively seeking a results-driven Director of Case Management to lead a well-established department at a full-service acute care hospital in New Mexico. This is an excellent opportunity for an experienced RN leader to step into a high-impact role focused on patient transitions, quality outcomes, and cost-effective care.
Position Overview:
Reporting directly to the CFO, the Director of Case Management will oversee a team of approximately 20 professionals, including RNs, Social Workers, and support staff. The role is responsible for managing the full spectrum of case management services across the patient continuum—ensuring efficient utilization of resources, timely transitions of care, and optimal patient outcomes. This leader will also have oversight of the hospital’s Patient Advocate program.
Key Responsibilities:
- Lead all case management operations, including utilization review, discharge planning, and transitions of care
- Develop and implement departmental goals aligned with quality and financial performance metrics
- Supervise and mentor staff across clinical and non-clinical roles
- Monitor and improve workflows, compliance, and program effectiveness
- Ensure coordination across departments for effective patient care planning
- Oversee Patient Advocacy initiatives across all age groups (newborn through geriatrics)
Required Qualifications:
- Current RN license in the State of New Mexico or compact license
- Minimum of 3 years’ experience in case management and utilization review
- At least 2 years in a leadership or management role within a healthcare setting
Preferred Qualifications:
- Certification as a CCM (Certified Case Manager) or ACM (Accredited Case Manager)
- InterQual or NAHCP certification
