Mary Free Bed at Home seeks a compassionate Home Health Medical Social Worker to join our team of dedicated professionals. As a medical social worker, you'll collaborate with our team to provide management and delivery of patient care plans.

To be successful in this role, you'll need:
- A Master's in Social Work from graduate school accredited by the Council of Social Work
- One (1) to two (2) years post-graduate social work experience in a healthcare setting
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
- Current driver's license and ability to spend ~20% of your day driving to/from patient locations
- A commitment to consistently meet critical deadlines for charting
- The skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones, and EMR software
