MH/SUD Case Manager (CDCA) Job Summary:
- We are seeking a skilled and compassionate MH/SUD Case Manager to join our team at Maryhaven, Central Ohio's largest and most comprehensive behavioral services provider.
Key Responsibilities:
- Under the direction of Site Supervisor, provide substance use disorder and mental health case management and treatment planning for youth and adults referred to Maryhaven for treatment.
- Develop and implement individualized treatment plans that address the unique needs of each client.
- Conduct assessments, evaluations, and progress monitoring to ensure clients receive high-quality care.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
- Provide education and support to clients and their families on addiction recovery and mental health topics.
Requirements:
- Bachelor's degree in human services or behavioral healthcare field, or associate's degree with some experience in chemical dependency treatment.
- State of Ohio Chemical Dependency Counselor Assistant (CDCA) certification required.
