Responsibilities
- Conducts outreach to Medicaid recipients and network providers to remind and promote utilization of preventive services.
- Finds and/or creates preventive and educational opportunities and events through research and collaboration with interdepartmental, local, state, and federal entities to build an ongoing calendar of events serving members and providers. This includes determining appropriate activities to attend to provide health education based on P&W Work Plan goals.
- Targets phone calls to Medicaid recipients without recent visits with a healthcare practitioner.
- Identifies barriers to accessing healthcare services and takes appropriate action to address such obstacles (i.e., appointment scheduling, coordination of transportation services) by making referrals to Care Management.
- Educates Medicaid recipients on the importance of preventive healthcare through virtual, telephonic, and onsite health education presentations, informational tables, health fairs, community baby showers, and other venues as needed.
- Supports quality initiatives and regulatory, contractual, and/or accreditation requirements including but not limited to NCQA, EQRO, and HEDIS.
- Maintains and updates a resource library of educational programs/resources and develops materials/tools for specific sponsored programs, events, and Community Advisory Board meetings.
- Tracks and reports on outreach and educational activities and monitors outcomes of educational presentations through surveys every month and as needed by supervisor.
- Assists with coordinating the production of the quarterly member and provider newsletters and assists with writing and reviewing articles as applicable.
- Other duties and/or projects as assigned, including but not limited to multiple meetings.
- Possession of a valid Maryland State Driver’s License is a requirement for this position.
Knowledge, Skills, and Abilities:
- Background in managed care, case management, or population health.
- Effective communication skills, public speaking skills, and influencing skills that can be applied to a broad internal/external audience.
- Ability to successfully utilize Microsoft Office suite.
- Ability to be professional, take direction and constructive feedback, work collaboratively with supervisor, and work as part of a team.
- Strong problem-solving, organizational, and time management skills.
- Ability to work in a fast-paced environment with changing priorities as required.
- Must have own reliable transportation with appropriate insurance and willingness to do statewide travel.
- Associate or bachelor’s degree in a health-related field.
- Demonstrated experience in a managed care environment with a minimum of one year of recent experience with health education, outreach, or working with local health departments, agencies, and/or organizations in underserved communities.
- Experience with public speaking, creative, writing, and graphic arts experience is preferred.
