What are the responsibilities and job description for the Qualified Intellectual Disability Professional (QIDP) position at maryleefoundation?

Purpose:      Mary Lee Foundation is a 501(c)(3) nonprofit organization dedicated to providing diverse services and housing to adults with various types of disabilities. We are looking for a Qualified Intellectual Disabilities Professional to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.

 

Responsibilities: 

·         Coordinates admissions and discharges of residents and prepares all appropriate documentation.

·         Facilitate the interdisciplinary team (IDT) process in conjunction with all residents assigned to caseload.

·         Facilitate the annual staffing of individual residents to develop and monitor treatment plans and Individual Program Plans (IPPs).

·         Provides liaison between the resident, the Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), the family and the community.

·         Assist family and resident in dealing with adjustment to placement and interpersonal relationships with other residents and other issues in the person's life.

·         Is aware of available resources in the Austin community and provides information and makes referrals to specific services as needs of the client and family indicate.

·         Create or work with behavior specialist on Behavior Modification Plans (BMPs) in support of clients who display challenging behaviors.

·         Stay abreast of current best practice to provide optimal support for clients.

·         Facilitates family involvement and encourages communication between family and resident.

·         Performs necessary documentation, record keeping, report writing and any correspondence in required timely manner and in accordance with established procedures and incompliance with state standards such as but, not limited to; annual staffing reports, IPPs, IDRCs (waiting to receive training and access to TMHP) ICAPS (also need training and necessary software) and monitoring of progress notes.

·         Develop goals and objectives for a resident’s IPP, monitor all areas of training, and enforce any IDT recommendations as determined in the annual staffing by the IDT.

·         Assist residents in vocational placement, volunteer work, schooling, counseling or needed training.

·         Assist in maintaining all residents’ clinical records, by keeping charts current, accurate and orderly.

·         Monitor resident’s special requests, budgeting needs and coordinate with Bookkeeper to maintain resident’s trust fund.

·         Assists with planning leisure activities for residents on caseload.

·         Supervise Direct Care Staff who work with residents on caseload.

·         Coordinates with nurse to ensure that all client medical needs are taken care of in a timely manner.

·         Follows on call schedule to assist staff and residents with problems that occur after regular business hours.

·         Works direct care shifts as needed when on call.  Must be able and willing to perform all duties on the Direct Care Staff Job Description.

·          Adhere to professional standards as outlined by protocols, rules and regulations.

·         On call one full week every four weeks.

*  Responsibilities are not limited to the above list and may include other duties as deemed necessary by supervisor.

 

Knowledge, Skills and Abilities: 

·      Excellent knowledge of case management principles, healthcare management and reimbursement

·      Previous experience with psychological aspects of care

·      Effective communication skills

·      Excellent organizational and time management skills

·      Familiarity with professional and technical emerging knowledge

·      Problem solving skills and ability to multi-task

·      Compassionate with teamwork skills

 

Qualifications: 

·      2 years experience working with people with disabilities

·      Supervisory experience preferred.

·      Bachelor’s degree in a human services related field (i.e., social work, special education, psychology, rehabilitation counseling, or sociology)

·      Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job

·      Good criminal history

·      Valid Drivers License

·      Clean driving history

 

Physical Requirements:

With or without reasonable accommodation, the physical and mental requirements of this job may include the following: seeing, hearing, speaking, and writing clearly. Occasional reaching with hands and arms, stooping, kneeling, crouching, crawling, frequent sitting, standing and walking, may be required for long periods of time and may involve climbing stairs, walking up inclines, and on uneven terrain. Additional physical requirements may include, frequent lifting, bending down, and/or moving up to 50 pounds.
