Your Role: As a Housing Stability Case Manager, you will play a vital role in guiding families and youth (ages 16-22) on their path to long-term stability and housing security. Using a trauma-informed, person-centered, and wraparound approach, you will:
- Build Trusting Relationships: Establish rapport with clients to understand their unique strengths, barriers, and needs.
- Develop Personalized Plans: Create individualized housing stabilization plans tailored to each client's needs, drawing on your expertise and community resources.
- Connect Clients with Resources: Facilitate access to essential services such as housing assistance, employment resources, educational opportunities, healthcare, and mental health services.
- Empower Clients to Thrive: Provide ongoing support, advocacy, and mentorship to help clients develop life skills, build self-sufficiency, and maintain stable housing.
- Strengthen Community Partnerships: Collaborate effectively with landlords, property managers, and community-based organizations to ensure clients have access to comprehensive support.
What We Are Looking For:
- Education: High School Diploma or GED required. A college degree (2 or 4-year) in Social Work, Human Services, or a related field is preferred.
- Experience: Minimum 2 years of direct experience working with youth and/or families in the areas of housing instability, homelessness prevention, or related social services. Experience in case management and crisis intervention is highly desirable.
- Certifications: Community Health Worker (CHW), Traditional Health Worker (THW), or Qualified Mental Health Associate (QMHA-I) certification is a plus - we can help you obtain one!
- Key Skills:
- Strong understanding of trauma-informed care, youth/family development, and housing-first practices.
- Excellent interpersonal skills and the ability to build trusting relationships with individuals facing challenging circumstances.
- Ability to work independently, manage time effectively, and balance multiple priorities.
- Strong communication, collaboration, and advocacy skills.
- Organized, detail-oriented, and capable of maintaining accurate records.
