Position Summary
Responsible for providing strength-based home visitation services through the Healthy Families America (HFA) model to pregnant people and parenting families. Provides case management services consisting of completing assessments, presenting evidence-based health, parenting, and child development education and providing referrals and other supportive services. Nontraditional working hours are required based on operational needs.
Position Responsibilities
- Provides intensive case management services including assessments, referrals, home visit documentation, and follow-up to at risk pregnant people and parenting families through weekly home visits and regular phone contact
- Supports families experiencing risk factors including trauma (current or historical), substance use disorder, IPV / DV, emotional wellness and mental health diagnosis, and child welfare involvement.
- Carries a caseload of 15-20 families with high levels of need
- Conducts health promotion activities by providing evidence-based health education, disseminating early childhood education literature, and distributing reproductive health information and materials
- Provides strength based, trauma informed, culturally inclusive parenting support through evidence informed curriculum
- Supports families in planning for their own health and social service needs by developing Family Service Plans (FSP), Goal Plans (GP), and Plans of Safe Care (POSC) Participates in orientation and ongoing professional development opportunities on topics pertinent to service communities such as maternal and child health, child abuse reporting / prevention, accessing resources (medical care, therapeutic services, public benefits, housing, mental health, substance abuse), parenting support, and DEIJB (out of State travel may be required)
- Keeps accurate records of services received by documenting all referrals, family and provider contacts within 72 business hours.
- Learns about resources within the community to support families
- Routinely attends outreach events in the community (i.e. community meetings, health fairs, job fairs, tabling at community events, etc.) as requested by Program Manager.
- Ensures establishment of care and scheduling of prenatal, postpartum, well child, and early intervention appointments
- Assists in organizing family meetings, parenting groups and other socialization events
- Routinely uses case management databases (ETO, SharePoint, TS Gold) and other systems including Microsoft Office Suite
- Meets with Program Manager weekly for reflective supervision
