We are seeking a skilled Adult/Geriatric Care Provider to join our team for Home Risk Assessments. As a Matrix Provider, you will conduct thorough assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
You will work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate medical personnel as necessary. Your services will include patient education, follow-up and providing care in various venues, including Home Visits and Skilled Facility Visits.
Key Responsibilities:
- Conduct Adult/Geriatric Assessments

Requirements:
- Strong computer skills and familiarity with Employee health/medical record software
- Comfortable and flexible with frequent change
- Able to travel and possess a valid state driver license and adequate automobile insurance coverage
