Job Description
Case Manager - Family Support
I. GENERAL DESCRIPTION : The Case Manager - Family Support is an integral part of the Family Support program of the Mattie Williams Neighborhood Family Center, and provides outreach, advocacy and family assistance to residents of Safety Harbor, Oldsmar and Eastern Clearwater. The Case Manager - Family Support works directly with the front-line family support staff, interns, and volunteers to identify and serve clients based on the urgency of their needs. The Case Manager - Family Support carries a full case load of families, individuals, and youth in need of support to achieve their maximum potential, with a focus on the strengths and abilities of parents and use goal-setting to help build their capacity in education, job skills, employment, and financial stability, and to improve long-term family sustainability through coaching to provide a supportive, stable, and encouraging environment. The Case Manager - Family Support is a full-time, non-exempt employee, with a flexible 40-hour work week to include some evenings and occasional weekends as needed.
II. REPORTS TO : Program Director Status : Non-Exempt
III. DIRECT REPORTS : Interns, Volunteers, shared support of Family Support staff
IV. RESPONSIBILITIES :
1. Family Assistance
a. Assess the needs of the families enrolled in programs and services offered by the Mattie Williams Neighborhood Family Center
b. Develop specific goal-oriented strategies to address a family’s needs
c. Interview clients and process emergency assistance fund requests
d. Maintain accurate files on families and individuals assisted
e. Report activities as required by funder(s)
f. Guide participants in setting a minimum of two goals and meet with them regularly to monitor and encourage progress
g. Advocate strongly on behalf of assisted families with both public and private agencies and businesses
2. Coaching
a. Work with qualified parents for 6 months to 1 year
b. Initiate contact with referred families and actively work to engage families in the program
c. Develop and maintain partnerships with Human Service Agencies
d. Conduct outreach activities and build client case load
e. Maintain participant files with all required documentation
f. Implement a career development program
3. Emergency Response / Affordable Housing
a. Respond quickly to the needs of families impacted by emergency situations including weather events;
b. Prioritize stabilizing families in safe, affordable housing to support healthy outcomes for parents and children, keep children in their registered school if possible, and ensure housing costs are sustainable within the family’s budget;
c. Develop emergency response plans to leverage the resources of the Center to include food, housing, financial coaching and assistance, and after-school support where appropriate.
4. Represent MWNFC at community events, educational fairs, coalition meetings, and strategy sessions
5. Other duties as assigned
V. REQUIREMENTS :
1. Ability to problem solve and take a strength-based approach with clients to encourage achievement of goals
2. Extensive knowledge and understanding of community resources and referrals
3. Proven experience coaching adults and helping them achieve goals
4. Be able to work independently and with a diverse staff
5. Ability to maintain client files and database in accordance with funder requirements
VI. Minimum Qualifications :
1. Bachelors degree in Human Services, Social Work, or related field
2. Two or more years experience in human services working directly with families
3. Experience and comfort with Microsoft Office suite
4. Excellent verbal and written communication skills and computer proficiency
5. Valid Florida Drivers License and proof of adequate insurance
6. Bilingual in English and Spanish preferred
7. Level II Background Screening required
