What are the responsibilities and job description for the Case Manager position at McIntosh Trail CSB?
Under general supervision, provides a broad range of paraprofessional social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties.
Essential Duties and Responsibilities:
Provides supports to adults diagnosed with mental illness or substance abuse disorders to improve functioning. Provides assistance in development of interpersonal community coping and functional skills – examples: teach skills to navigate social and family environments, personal medication monitoring, and self-management. Services are provided in community. Caseload of approximately 50 individuals. Documentations and productivity requirements. Daily documentation. Computer skills are required.
- 1. Individuals will be seen face to face- minimum two times per month and/or documented attempts/explanation as to why no attempt. 2. Maintains an accurate caseload of an average of at least 50 individuals per month. 3. Maintain a safety plan on each individual. Provide array of skill services per treatment plan/needs of the individual. Teach daily living skills; positive behavior management techniques; provide positive reinforcements for attempts/successes. As evidenced by chart review, Peer audits and clinical supervision.
- PSRI/Case Manager will maintain 55% standard of billable hours that are authorized, documented, and processed for billing according to protocol.
- Will report to clinical supervisor any issues involving clinical or ethical concerns. Will attend scheduled staff meetings to remain current on policies and protocols within the agency. Performance will be rated based on clinical supervisor reports and attendance records.
- Participates with Director in achieving/ maintaining a cohesive TEAM which functions at its most efficient level as evidenced by decreased employee turnover with stable employee commitment and favorable patient response.
- Perform related duties as assigned by supervisor.
- Maintain compliance with all company policies and procedures.

- Demonstrates knowledge of staff and organization code of ethics and knowledge of and ability to follow AIDS/Infection Control guidelines.
Education and/or Work Experience Requirements:
Bachelor's degree in a social service related field from an accredited college or university OR High school diploma or GED AND Two years of experience in a social services related position OR One year of experience at the lower level Social Svcs Tech 1 (SST010) or position equivalent
