What are the responsibilities and job description for the Nurse Navigator - GU/Head and Neck Nurse Navigator position at McLaren Health Care?
Provides direct patient care and comprehensive coordination of care for patients and their families/caregivers. Eliminates barriers to timely care, facilitates flow through system, increases patient and provider satisfaction, maintains point of contact with providers and serves as the first point of contact for patients and families. Provides a full range of nursing services, including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with professional practices and ethical standards.

- Provides direct patient nursing care for specified patients
- Performs nursing assessments and triage of patient care needs for new and ongoing patients.
- Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
- Central point of contact for all navigated patients; including interaction with medical, nursing, ancillary services.
- Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public.
- Collects and reports data from patient on barriers, health care, medications, prior treatments, etc.
- Tracks order completion and follow up. Monitors patient compliance with patient treatment plan.
- Facilitates all stages of the referral process to outside resources and care coordination as needed.
- Designs or participates in the creation of health education materials.
- Provides and documents quality, personalized and evidence-based care and interventions.
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. May develop and conduct in-service training programs.
- Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. This includes compliance with all health care regulations, such as HIPAA and OSHA.
- Performs other related duties as required and directed
- Nursing degree from an accredited educational institution.
- State licensure as a registered nurse.
- Bachelor's degree in nursing.
- One year prior work experience in clinical area.
- BLS certification.
