Position Summary
The Case Aide performs a variety of tasks related to the rendering of appropriate paraprofessional social services and related work as assigned. Work is performed under the day-to-day direction of social workers and oversight of a social service supervisor.

Essential Functions / Responsibilities

Position Duties
Review case records for completion of required agency forms (case file reviews)Complete, manage and update approval option providers / process.Complete intake daily as assigned, take MnCHOICES referrals and other intake calls, explain intake process to Provide data privacy form and inform clients of the right to appeal and of other rights as needed.Make appropriate referrals to the other HHS units and / or other communityContact community resources as needed andPerform clerical / technical functions as assigned and related to social services team.Perform statistical operations relevant to social service programAssist the team in processing purchase of service contracts, SSIS services agreements and voucher and MMIS entry.Compose correspondence and construct reports asAssist social workers / case managers with administrative activities related to waiver case management services.Non-Essential Functions

Education and Experience
Three (3) years of successful clerical experience and / or experience working with the public, or theOr two (2) years of successful clerical experience in a social service / human services agency;Or two (2) years of experience as a community service aide or similar experience in a private non- profit agency or other public agency;Or two (2) years of study at an accredited two (2) or four (4) year college / university or similar institution, with emphasis in the behavioral sciences (at least 23 quarter credits or 16 semester credits)Required Certifications / Licenses
Valid Minnesota driver's license with proof ofRelationships
