Provides protection and case management services for Vulnerable Adults; conducts time sensitive investigations into allegations of abuse, neglect, and exploitation.

  • Pay range shown is hiring range. Full range goes up to $113,797 with outstanding performance.

Essential Duties

Conducts thorough assessments/investigations within required timeframes:
  • Interviews and obtains statements from Vulnerable Adults, person(s) alleged responsible, and potential witnesses concerning the allegations of abuse, neglect, and exploitation
  • Conducts independent home visits and interviews, both scheduled and unannounced
  • Completes Safety Assessments, Strength and Needs Assessments, and Safety Plans when applicable
  • Completes required documentation, writes, and enters clear and organized data for reports, and summarizes the facts to determine whether abuse, neglect, or exploitation has occurred
  • Collects pertinent evidence such as progress notes, medical records, financial records, photographs, and physical evidence
  • Analyzes complex financial and medical documentation
  • Makes findings, both independently and in consultation with a team, regarding allegations of abuse, neglect and exploitation
  • Collaborates with law enforcement, county attorney, and other community partners
  • Provides court testimony and/or shares written findings with the courts
  • Determines which reports require immediate adult protective services and initiates a response
  • Understands Vulnerable Adult Law, conservator/guardianship laws, applicable criminal statues, and has the ability to apply these laws to case situations
  • Maintains working knowledge of federal and state laws and regulations and performs work accordingly

Provides Protection Services:
  • Makes independent decisions in the field regarding protection and safety
  • Initiates interventions including calling 911, coordinating with law enforcement and/or medical professionals, engaging family, friends and informal supports, referring for services, etc.
  • Arranges for court ordered interventions, such as guardianship or conservatorship, and attends court hearings when required
  • Mobilizes community resources to provide protective services to remedy the reported maltreatment and reduce the risk of further abuse, neglect or financial exploitation

Provides Case Management Services:
  • Visits clients in their home or other residence in the community
  • Coordinates and offers community resources to address needs and reduce risk
  • Serves as primary point of contact for service coordination and sharing of information

Desirable Skills:
  • Excellent oral and written communication
  • Familiarity with the challenges faced by aging and disabled individuals
  • Knowledge and experience working with complex mental health needs including Substance Use Disorder and Hoarding
  • Knowledge of crisis intervention skills, techniques, and resources
  • Knowledge and experience working with MnCHOICES, Waiver programs, Medical Assistance, Medicare, and the Department of Veterans Affairs
  • Understanding of, or a willingness to learn about, diverse cultures and values
  • Ability to analyze complex financial documents and contracts
  • Emotional capability to manage unsettling allegations of abuse or neglect
  • Ability to operate a personal computer and various software packages
  • Demonstrated ability to engage in critical and objective thought
  • Knowledge of Supported Decision-Making practices
  • Basic understanding of Guardianship and Conservatorship laws, Power of Attorney, Health Care Directives, and Representative Payee

Attends staff meetings and participates in ongoing training.

Works the days and hours necessary to perform all assigned responsibilities and tasks. Must be available (especially during regular business hours or shifts) to communicate with peers, supervisors, customers, vendors, and any other person or organization with whom interaction is required to accomplish work and employer goals.

Performs other related duties as required or assigned.

Minimum Qualifications

Requires a minimum of a bachelor’s degree in social work, psychology, nursing, human services, or other related field and at least two years of professional social work experience working directly with clients. Strong preference for experience within Adult Protection. Master's degree and bilingual abilities are desirable. Must possess a valid driver's license and provide a reliable means of transportation for the performance of work responsibilities.
