**Job Summary:**
We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Medical Flex RN - Care Coordinator. In this role, you will provide direct care to our residents, collaborate with interdisciplinary teams, and ensure seamless coordination of care.
**Key Responsibilities:
Care Coordination: You will be responsible for assessing resident needs, developing individualized care plans, and implementing interventions to improve outcomes.
