Job Summary
We are seeking a dedicated and compassionate Registered Nurse Certified Advantage Case Manager with experience with the OKDHS Advantage program to join our healthcare team in a supervisor role The ideal candidate will play a crucial role in coordinating member care and supervising personnel and directing case management and nursing/ home care services through the Advantage program ensuring that individuals receive the necessary services and support with their in home care. This position requires strong clinical and organizational skills, effective communication, and a commitment to providing exceptional supervision to employees and the Advantage members in the in-home long-term care population setting.
Responsibilities
- Assign and supervise care provided to members in the Advantage program for Case Mangement, home care or SPPC services through OKDHS.
- Coordinate care across various departments, including assigning cases as appropriate, monitoring of employees documentation and pay roll.
- Collaborate with interdisciplinary teams to ensure continuity of care and optimal member outcomes.
- Reviewing members plans developed by case managers to ensure that members plan is appropriate for their needs according to MSU policy and guidelines
- Provide education and resources to Members and families regarding their person centered plan of care.
- Advocate for members needs within the program assigned while maintaining compliance with regulatory standards.
- Document all interactions and interventions in accordance with established policies and procedures.
Experience
- Current Registered Nurse (RN) license in good standing.
- Previous experience in Advantage case management or a similar role is preferred.
- Supervisor role in the aged population of frail elderly and or physically disabled population.
- Strong organizational skills with the ability to manage multiple cases simultaneously while maintaining attention to detail.
- Excellent interpersonal skills to effectively communicate with Members, families, and healthcare professionals.
- Proficiency in computer/ Harmony system skills.

Pay: $28.00 - $32.00 per hour
