Patient Population : Hospice home care patients
Setting : Hospice Home Care
Responsibilities : The majority of the hospice physician's time is spent in interdisciplinary team meetings, reviewing cases for hospice eligibility, and providing guidance and recommendations to the nurses in the field. Home visits for face-to-face encounters and symptom management are scheduled as needed.
Required Procedures : None specified
EMR System : EPIC - Comfort / Hospice Application
Support Staff : Hospice nurses and interdisciplinary team
Days Per Month : Full-time, Monday-Friday, 8am-4 : 30pm
On-Call / Weekends : On-call is 1 weeknight per week and 1 in 4 / 5 weekends (Friday 4pm to Monday 8am)
Submission Requirements :
- Board Certified in Internal Medicine, Family Medicine, or Emergency Medicine (required)
- Hospice and Palliative Medicine or Geriatrics Fellowship (a plus)
- NY State License (required)
- BLS Certification (required)
- Provider must be local (required)
- Conversion Rate Cap of 25k (required)
