Responsibilities
Your key responsibilities will include:
- Performing comprehensive assessments and developing plans of care in collaboration with physicians and other healthcare professionals
- Regularly evaluating and re-evaluating patient needs, as warranted by changes in condition but at least every 60 days
- Ensuring patients meet home health eligibility and medical necessity guidelines as defined by payer sources
- Initiating, developing, implementing, and making necessary revisions to plans of care in collaboration with physicians and other healthcare professionals involved in care
Requirements
To be successful in this role, you will need:
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
