**Travel 3 Days A Week For Home Visits**
**Must Have Experience with Aging, Blind, Disabled Population**
**LVN or Related Experience**
**Spanish Speaking a Plus**
Day-to-day responsibilities of this the role and a description of the project:
- Assist in telehealth biannual assessments, in person as needed if in the area.
- Complete assigned tasks and document findings.
- Coordination with multi-disciplinary teams.
- Follow up calls to members.
- Any other projects that may arise.
Performance and Expectations / Metrics for this individual and their team:
- Must be able to meet strict turn-around times with documentation for assessments assigned.

- Licensed vocational nurse
Skills/ Experience:
- Licensed vocational nurse (LVN) or Related Experience
- Bilingual preferred
- Organized, Strong time management skills
- At least 2 years’ experience
Salary : $24 - $25
