  • Day-to-day responsibilities of this the role and a description of the project : Assist in telehealth biannual assessments, in person as needed if in the area.Complete assigned tasks and document findings.Coordination with multi-disciplinary teams.Follow up calls to members.Any other projects that may arise.Performance and Expectations / Metrics for this individual and their team : Must be able to meet strict turn-around times with documentation for assessments assigned.Address tasks in a timely manner.FlexibleProfessionalWhat previous job titles or background work will in this role : Licensed vocational nurseSkills / Experience : Licensed vocational nurse (LVN) or Related ExperienceBilingual preferredOrganized, Strong time management skillsAt least 2 years’ experience
