Tonawanda Pediatrics: a division of the Medical Health Associates of WNY, is looking for a dedicated Behavioral Health Care Manager to join our collaborative care team! In this role, you'll work alongside medical providers and psychiatric consultants to ensure comprehensive care for our patients.
What You'll Do:
- Support & Coordinate: Manage the mental and physical health of 80-100 patients through phone, video, or in-person interactions.
- Assess & Engage: Screen patients for behavioral health disorders and facilitate ongoing care and education about treatment options.
- Monitor Progress: Track treatment responses and adapt plans as needed, ensuring effective care and medication management.
- Provide Interventions: Utilize evidence-based techniques like cognitive behavioral therapy and motivational interviewing.
- Collaborate: Facilitate weekly consultations with psychiatric consultants and communicate treatment plans to medical providers.
- Document & Innovate: Keep accurate records in our EHR and identify patients who may need additional support.
Requirements
Who You Are:
- Licensed Mental Health Counselor, Licensed Marriage and Family Therapist, Licensed Social Worker or Masters-level licensure candidate/trainee (e.g. MSW).
- A collaborative communicator with experience in behavioral health assessment and treatment planning.
- Skilled in engaging patients and providing compassionate care through various formats.
What We Offer:
- Competitive salary range: $52,000 - $60,000, based on licensure and experience.
