Job Title : Case Manager - Medicaid Long Term Care / Comprehensive Program

Position Type : Full-time, Monday-Friday (8 : 00 AM - 5 : 00 PM)

Position Summary :

We are seeking a dedicated and compassionate Case Manager to provide comprehensive healthcare management services for Medicaid Long Term Care / Comprehensive Program enrollees. The Case Manager will facilitate appropriate healthcare treatment, manage healthcare costs effectively, and improve operational efficiency. The role will involve case coordination, support, and education to ensure positive healthcare outcomes for members.

Key Responsibilities :

Care Coordination & Case Management :

Coordinate case management activities for Medicaid Long Term Care / Comprehensive Program enrollees.

  • Conduct comprehensive evaluations using care management tools and review data / information to facilitate decision-making and care planning.
  • Implement and monitor care plan activities, ensuring progress is tracked and goals are met.
  • Collaborate with multidisciplinary teams to achieve optimal healthcare outcomes.
  • Provide support and guidance to members, ensuring they actively participate in their healthcare decisions.

Healthcare Advocacy & Member Support :

Utilize negotiation and motivational interviewing skills to secure appropriate services and engage members in their care.

  • Empower members to make informed, independent healthcare and lifestyle decisions through coaching and education.
  • Address quality of care issues, escalating as necessary to ensure members' needs are met effectively.
  • Documentation & Compliance :

    Ensure compliance with regulatory, accreditation guidelines, and internal policies while maintaining comprehensive case documentation.

  • Participate in continuous quality improvement processes and adhere to case management best practices.
  • Qualifications : Experience :

    Minimum of one year of case management experience is required.

  • Experience in long-term care is preferred.
  • Case Management Certificate (preferred).
  • Proficiency in Microsoft Office, including Excel.
  • Bilingual in Spanish / English is preferred.
  • Education :

    Bachelor's degree required (preferably in Social Work or a related field).

  • Nursing degrees are not applicable to this position.
  • Skills and Attributes :

  • Strong critical thinking and judgment to effectively evaluate and manage care plans.
  • Excellent negotiation and motivational interviewing skills to influence and engage members.
  • Ability to assess member needs and tailor care coordination strategies accordingly.
  • Ability to work collaboratively within a multidisciplinary team.
  • Working Hours :

  • Monday to Friday, 8 : 00 AM - 5 : 00 PM (with possible variations during training).
