Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients and their families to develop individualized care plans that address their unique needs, promoting independence and enhancing quality of life.
Shift: 7:00 AM - 3:30 PM
Floating: Potential to float to other territories to assist when needed during low census
Patient Ratios: 20-25 patients, averaging 4.5 visits per day
Certifications: WI License, AH ABLS
Experience: Minimum 1 year of home health experience required, 2 years strongly preferred; travel experience preferred
Duties: Oasis admissions, all time points, episode management, physician communication and coordination of care, timely EMR documentation, clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management
Charting System: EPIC and OASIS (experience strongly preferred)
