Title: Family Nurse Practitioner
Day to Day:
- Conduct thorough patient assessments, including medical history reviews, physical examinations, and risk factor evaluations.
- Provide accurate diagnosis and treatment of acute and chronic illnesses in alignment with best medical practices.
- Order, interpret, and analyze medical diagnostics such as laboratory tests, imaging, and other assessments to inform treatment decisions.
- Develop and manage personalized treatment plans tailored to each patient's condition, considering evidence-based guidelines and patient-specific needs.
- Offer chronic disease management services, including ongoing monitoring and coordination of care for conditions such as diabetes, hypertension, and cardiovascular diseases.
- Promote preventative care and wellness strategies, including immunizations, screenings, and lifestyle modifications.
- Prescribe medications, treatments, therapies, and medical equipment in accordance with regulatory and clinical guidelines.
- Educate patients and families on self-care strategies to enhance long-term health outcomes and encourage proactive healthcare management.
- Oversee and ensure the proper operation of onsite CLIA-waived laboratory testing and procedures.
- Dispense prepackaged medications onsite while adhering to appropriate prescribing and dispensing protocols.
- Coordinate referrals to external specialists and healthcare providers as necessary for comprehensive patient care.
- Deliver presentations on various health-related topics upon request to support community education and outreach initiatives.
- Provide clinical leadership, ensuring the highest standards of patient care, collaboration among healthcare teams, and adherence to ethical and professional standards.
Qualifications:
- Unrestricted Nursing License to practice in Minnesota
- Family Practice or Advanced Practice Nurse Certification by ANCC or a graduate of an accredited Physician Assistant program
- Minnesota Prescriptive authority and DEA licensure
- BLS certification; ACLS suggested
- Must be comfortable working independently

- Monday-Thursday 7:30-5:30pm
