They will serve as the main point of contact for families and residents and be responsible for the overall management of the facility. Additionally, the role includes acting as an abuse and grievance counselor in collaboration with administration for required reporting.Responsibilities / Job Duties : Review care plans and new information upon arrival each dayReturn all phone calls related to care and facility needsPrepare quarterly reports and update care plansCoordinate and conduct interviews for new admissionsCollaborate with hospitals for discharges (primarily for hospital transfers)Work closely with residents, as most are long-term / lifetime residentsMinimum Education and Experience Qualification Requirements : Licensed Independent Clinical Social Worker - LICSW (Required)Experience working with long-term care / elderly patient populations requiredExperience establishing care plans requiredExperience with MDS process for clinical assessment (Medicare / Medicaid patient populations in care facilities) requiredMust be comfortable working in an environment with Christian affiliationsFor California Applicants : We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO) , and the California Fair Chance Act (CFCA).
