Job Title: Regional Community PrEP Navigator
Program/Service: PrEP Navigation; Community Health Worker
Location: Downtown Chicago
Hours: Monday-Friday Day shift (flexibility with nights and weekend for events)
JOB SUMMARY: The Regional Community PrEP Navigator (RCPN) provides PrEP navigation and referral services, expanding access to high-risk clinics. Responsibilities include STI/HIV counseling, prevention education, and connecting vulnerable individuals to PrEP services. The role also involves evaluating health education sessions, ensuring PrEP adherence, and participating in trainings and administrative meetings.
Provide PrEP Awareness, Community Outreach, and Recruitment
- Collaborate at community events to offer HIV testing, PrEP education, and referrals.
- Initiate contact with interested patients and complete intake forms for PrEP linkage.
Conduct HIV Testing, PrEP Education, and Referrals
- Perform HIV testing and provide counseling, linking HIV-positive individuals to care.
- Assess participants for PrEP eligibility and document all services in required databases.
Maintain Accurate Charting and Documentation
- Document patient encounters and referrals in the Electronic Medical Record (EMR).
- Ensure compliance with documentation policies, including service referrals and approvals.
Provide Client-Centered PrEP Adherence Counseling
- Offer health education, risk reduction strategies, and assist with PrEP initiation.
- Refer patients for insurance assistance and assist with Medication Assistance Program (MAP) applications.
Assist in Developing Peer Educators
- Train and mentor peer program members to become PrEP peer educators and navigators.
- Provide guidance to high-risk individuals to support community outreach and education.
QUALIFICATIONS:
- High school diploma or equivalent required; bachelor’s degree in a health-related field preferred.
- 1-3 years of experience in HIV and PrEP counseling or education required.
- Experience with multiple HIV rapid tests is a plus.
- Ability to work with diverse populations.
- Valid driver’s license and car insurance required.
- Strong oral and written communication skills.
- Proficiency in computer skills and data entry.
- Ability to work evenings and weekends as needed.
- Strong organizational and deadline management skills.
- Willingness to work as part of a multidisciplinary team.
