Position Overview:
We are looking for a bilingual (Spanish) Collaborative Care Social Worker (LMSW/LCSW) with the ability to handle a high case load! They will work closely with a multidisciplinary team that includes primary care providers, psychiatrists, and other mental health professionals to provide care and support for patients managing anxiety and depression. This role requires strong clinical skills in mental health and a passion for helping patients improve their quality of life through evidence-based practices. The ideal candidate is comfortable working in a collaborative environment, communicates effectively with team members, and is adept at implementing individualized interventions to meet each patient's unique needs.
Key Responsibilities:
- Provide Assessment and Intervention: Conduct comprehensive psychosocial assessments to determine patient needs and develop personalized care plans. Provide evidence-based therapeutic interventions for anxiety and depression, including Cognitive Behavioral Therapy (CBT), Behavioral Activation, and mindfulness techniques.
- Collaborative Care Coordination: Work closely with primary care providers, psychiatrists, and other healthcare professionals to ensure continuity of care. Serve as a liaison between patients and providers, ensuring adjustments to treatment plans based on patient progress and feedback.
- Patient Support and Education: Provide patients and their families with education on anxiety, depression, coping strategies, and available resources to empower self-management. Offer supportive counseling and psychoeducation to facilitate recovery and resilience.
- Monitoring and Follow-up: Regularly assess patient progress, conduct follow-up assessments, and adjust interventions to support patient goals. Utilize measurement-based care tools such as the PHQ-9 and GAD-7 to track treatment response.
- Documentation and Compliance: Maintain detailed, accurate documentation of patient encounters and treatment plans in compliance with legal and organizational standards.
- Outreach and Advocacy: Advocate for patients' access to services, including referrals to specialized providers and community resources. Educate the community about mental health and work to reduce stigma surrounding anxiety and depression.
Required Qualifications:
- Education: Master’s degree in Social Work (MSW) or a related field.
- Licensure: Current New York State licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- Experience: A minimum of 2 years of clinical experience working with populations experiencing anxiety and depression, ideally within a collaborative care or integrative care environment.
- Skills:
- Strong knowledge of evidence-based treatments for anxiety and depression, including CBT and mindfulness strategies.
- Excellent interpersonal and communication skills, with the ability to work effectively in a multidisciplinary team.
- Proficiency with Electronic Health Records (EHR) systems.
- Ability to manage a high caseload while maintaining quality of care.
- Fluency in Spanish is required to serve a diverse patient population.
