A Long-term care group is in search of a Primary Care Nurse Practitioner to see geriatric patients in nursing homes located in the five towns.
Requirements:
- Valid NP license in the state of NY
- Either of the following experience needed: Critical Care, Emergency Room, Geriatrics or Nursing Home
- Credentialed with Medicare- strongly preferred
Duties will include:
- Managing all Primary Care related consults
- History and physical exam
- Medication review
- Prescribe medication and drugs
- Check in with patients who have acute issues and establish a follow-up plan
- Reach an informed diagnosis based on medical knowledge and the patient's medical history
Schedule:
- 8-hour shifts
- Working days: Sunday, Tuesday, Thursday, Friday and Saturday
