Schedule: Hybrid REMOTE, M-F, 8am-5pm EST
1. in NOVA area with Behavioral Health experience for the SMI Team.
1. in Lynchburg with LTSS experience for the LTSS Team.
Summary: For this position we are seeking a Case Manager who live in VIRGINIA.- specifically in NOVA or LYNCHBURG.This is a fast-paced position and productivity is important.
TRAVEL (30% or more) in the field to do member visits in the surrounding areas will be required. Mileage will be reimbursed. Home office with internet connectivity of high speed required.
-Case Manager will work in remote and field setting supporting our Medicaid Population.
- Case Manager will be required to physically go to member’s homes to complete Face to Face assessment.
-You will participate in interdisciplinary care team meetings for our members and ensure they have care plans based on their concerns/health needs.
- Excellent computer skills and attention to detail are very important to multitask between systems, talk with members on the phone, and enter accurate contact notes. This is a fast-paced position and productivity is important.
Must Have Skills:
- Must have a valid driver’s license and reliable vehicle for field visits
- Must have Mental health/SMI experience and anyone with LTSS experience and conducting LOCERI assessments, if possible. NOVA, Central, and Roanoke areas.
- Excellent communication skills, with an ability to communicate effectively with members with chronic conditions, developmental disabilities, etc.
- Strong technical skills (utilizing multiple programs at the same time) along with competency in Microsoft applications (Outlook, Teams, Excel, etc.)
- Strong organizational and interpersonal skills
Day to Day Responsibilities: Will be responsible for performing case management activities according to the contractual requirements and in alignment with NCQA requirements. This will include potential home visits to complete assessments as well as completion of telephonic assessments, completion of integrated care plans, and conducting interdisciplinary care team meetings. In addition, conducting needed telephonic follow-up, education, and care coordination for assigned members.
Required Years of Experience: 1-3 years in case management, disease management, managed care or medical or behavioral health settings.
Required Licensure / Education: Must have valid driver’s license with good driving record and be able to drive within applicable state or locality with reliable transportation. Completion of an accredited Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) Program OR Bachelor's or Master's Degree (preferably in a social science, psychology, gerontology, public health or social work or related, Or Graduate from an Accredited School of Nursing. Bachelor's Degree in Nursing preferred
