Melmark is a nonprofit organization providing clinically-sophisticated evidence-based special education, residential, vocational, and therapeutic services for children and adults diagnosed with autism spectrum disorders, developmental and intellectual disabilities, acquired brain injuries, medical complexities, and other neurological and genetic disorders.
Case Manager Overview
Serves as the primary liaison for families, external interagency team members (funders, supports coordinators, care/case managers, school district representatives, county representatives, etc.), and clinical/program staff for the Residential Treatment Facility (RTF) licensed under the Department of Human Services (DHS) 3800 Regulations. The Case Manager is responsible for coordination of; (a) admissions, (b) interagency meetings, including preparation and submission of reauthorization packets, participation in regular phone contacts with managed care companies and the interagency teams, (c) scheduling and participating in family meetings, and (d) transition and discharge planning. Coordinates, and prepares and maintains required documentation to ensure compliance with internal and external regulatory standards and agency policy and procedures.

- Starting Salary: $50,000 ( ) based on years of direct relevant experience
Requirements
- Bachelor’s degree in social work, psychology or related field.
- A valid US driver’s license and a minimum age of twenty-one (21) years old. Successful completion of Melmark’s driving training.
- Prior experience working with Medicaid or other government funded programs and experience working with state personnel at all levels is preferred.
- A sound and thorough knowledge of behavioral health is required. Experience working with children and adolescents who have had multiple treatment failures and who require comprehensive clinical interventions is required.
- Experience in Applied Behavior Analysis is desirable.
Case Manager Responsibilities
- Attends admission meetings. Monitors and updates team on status referrals, pending admissions, upcoming graduates, and transfers via updated status list. Attends admission meetings. Assists and coordinates admission screenings for children and families referred to Melmark’s Residential Treatment Facility. Facilitates prospective referral visits on-site, off-site and in the family home, as appropriate.
- Coordinates admissions for children to Melmark’s Residential Treatment Facility job expectations
- Coordinates and participates in all meetings related to client care coordination including but not limited to clinical review, discharge planning, ISP, and interagency team meetings. Coordinates and processes all required documentation for interagency team meetings.
- Coordinates transition and discharge planning for residents. Schedules, attends, and facilitates interdisciplinary team discharge planning meetings. Works with the Melmark team, families, school districts, supports coordination organizations, and county and funding agencies to create discharge plans, including making necessary referrals, scheduling visits and screenings, coordinating overlap training, and obtaining authorizations. Completes all documentation related to discharge including discharge letters, discharge summary, and updating MEDS.
- Coordinates and tracks Therapeutic Leave recommendations, documentation, clinical case notes, and attendance.
- Responsible for reporting incidents to MCOs/Funders, including but not limited to restraints, emergency room visits, abuse claims, etc. as indicated by each MCO/Funder.
