Location : Diamondhead and Bay St. Louis (Hancock County)
Job Summary :
The Population Health Nurse will promote effective partnerships between patients, families, nurses, physicians, and other qualified healthcare providers and clinical disciplines to coordinate care for patients with chronic disease and effectively manage care transitions and facilitate a “shared goal model”. This nurse will partner with the provider care team for successful preventative care visits to reduce the severity of chronic disease and avoidable acute illness. They will provide effective clinical health coaching to assist patients with self-management of their chronic disease and life-style changes to mitigate health risk.
Required Qualifications :
Education : Graduate from an accredited, state-approved school of nursing.
Licensure : Current MS RN license, MS 90-day temporary permit, or Compact state RN license.
Experience : Two (2) years of experience in clinical nursing role caring for acute or chronic disease patients.
Skills, Knowledge, Abilities : Must possess strong clinical assessment and critical thinking skills necessary to develop a comprehensive plan of care appropriate for patients with complex medical, emotional and social needs. Must be patient / customer service oriented, projecting professionalism and socio-economic and cultural sensitivity. Ability to work in a high-volume caseload environment and manage rapidly changing priorities. Demonstrates evidence of effective organizational, leadership, communication, education, collaboration and counseling skills. Proficiency with electronic health records and other clinical applications.

Experience : One (1) year of experience in provider practice or clinic health setting. Previous experience in Care Coordination, Case Management, Home Health or Behavior Health.
Skills, Knowledge, Abilities : Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with diverse populations. Ability to speak a relevant second language. Proficient with data analytics.
