Job Summary
The Wound Care Coordinator is responsible for developing, implementing, and monitoring wound care plans for patients with chronic wounds. They work closely with nurses, physicians, and other healthcare providers to ensure that patients receive the best possible care and that their wounds heal as quickly as possible.
Essential Functions
- Conduct comprehensive wound assessments to determine the etiology, depth, and extent of wounds.
- Develop and implement individualized wound care plans based on patient needs and evidence-based practices.
- Select and apply appropriate wound dressings and therapies, including negative pressure wound therapy, hyperbaric oxygen therapy, and bioengineered skin substitutes.
- Monitor wound progression and document healing at regular intervals.
- Educate patients and their families on wound care techniques, self-management strategies, and signs of infection.
- Provide counseling and emotional support to patients and their families.
- Collaborate with nurses, physicians, and other healthcare providers to ensure continuity of care and achieve optimal wound healing outcomes.
- Stay up to date on the latest wound care research and clinical practice guidelines.
- Participate in quality improvement initiatives to enhance wound care outcomes.

Qualifications
- Minimum of three (3) years of experience in nursing with at least two (2) years of experience in wound care.
- Experience in a variety of wound care settings, such as hospitals, long-term care facilities, and home health agencies a plus.
- Experience in teaching and mentoring wound care nurses is a plus.
