ESSENTIAL FUNCTIONS AND BASIC DUTIES:

o Assesses, plans and implements care strategies that are individualized to the patient.
o Obtains consent to contact appropriate organizations and to participate in programs/services, as well as to
share information with the patient’s care team.
o Identifies and initiates referrals for social service and health care programs, including financial, psychosocial,
community and state supported services.
o Facilitates access to social and community services (ex. food assistance, transportation, housing/utilities, etc.),
behavioral health (ex. substance use disorder treatment and recovery), and health care services.
o Assists with applying for public benefits (ex. SNAP, TEFAP, TANF, WIC, etc.).
o Manages patient care plans throughout the continuum of care, as a single point of contact for the patient.
o Communicates the required social need and health-related information with appropriate stakeholders to
ensure high-level coordinated care and ensure services are provided expeditiously to patients.
o Maintains documentation of encounters with patients/clients and completes documentation
requirements according to department standards.
o Advocates for patients and families as needed to ensure patient needs and choices are fully represented
and supported by the care team.
o Builds supportive, collaborative relationships with patients, helping to empower, educate and assist them with needs.
o Serves as a community liaison and advocate by coordinating linkages and referrals to improve social, environmental,
and health conditions.
o Identifies and maintains a network of community resources and ancillary services (MRH-based and
external); participate in patient education and refer patients to appropriate resources.
o Some in-field visiting to patient/client homes or in the community.
o Participates in the development of standardized processes and procedures that create and support.
o Perform other duties as assigned. Occasional weekend work to support special events.

QUALIFICATIONS:
Minimum Requirements:
o Must be at least 16 years of age (21 for driving positions with a valid driver’s license).
o Must be able to legally work in the United States.
o Must be able to pass a background check.
o Must be able to pass a drug screen and breath alcohol test (if applicable).
o Must complete employee health meeting.
Required Education/Licensure/Certification:
o Bachelor’s degree or higher, preferably in nursing, social work, public health, or other healthcare- or
social services-related field, preferred but not required, OR
o Unencumbered Registered Nurse (RN), Licensed Practical Nurse (LPN), or Medical Assistant
(MA) license/certification in the State of Colorado, preferred but not required, OR
o Patient Navigation Certificate of Completion, through the Patient Navigator Training Collaborative
or Equivalent, at time of hire or within 6 months of hire.
Experience:
o Minimum 2 years’ experience in a health care, social service, or other relevant setting providing social
needs and/or other resource facilitation assistance, preferred.
Skills/Abilities:
o Apply National Patient Safety goals to care and documentation.
o Work within a Healthcare Electronic Medical Record to chart and review patient data, and run reports.
o Communicate effectively, both in writing and verbally.
o Establish and maintain effective working relationships with employees at all levels throughout the institution.
o Interpret, adapt, and apply guidelines and protocols.
o Identify and resolve problems in a proactive, collaborative manner.
o Commitment and leadership in regard to advancing diversity and inclusion.
o Initiative and self-motivation; strive to maintain steady level of productivity and good time management.

Compensation Range: $27.03 to $40.55
