Job Title: Home Care Coordinator
Senior Community Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.
Job Responsibilities:
- Oversight of day-to-day operations and operational effectiveness of the Home Care department for Senior Community Care.
- Participation in the Interdisciplinary Team (IDT) Meeting as required and representation as the Home Care Coordinator representative.
- Direct supervision of all home care Personal Care Attendants and Certified Nursing Assistants (C.N.A.s).
Qualifications:
- Current Licensed Practical Nurse license in the state of Colorado.
- Minimum of two (2) years' nurse management experience in a hospital, nursing home or community-based setting. Home health experience preferred.
- Broad knowledge base of healthcare problems of the frail elderly.
- Skill in preparing and maintaining records and writing reports and responding to correspondence.
- Skill in developing and maintaining department quality assurance.
- Ability to react calmly and effectively in emergency situations.
Key responsibilities include:
- Maintaining regular attendance at, and participating in Interdisciplinary Team meetings, communicating participant changes, collaborating on plan of care decisions and coordination of twenty-four (24) hour care delivery.
- Maintaining participants' medical record and fulfilling Senior Community Care charting and reporting requirements.
- Providing health counseling to participants and their families on an individual or group basis as needed.
- Engaging in team building, coaching, mentoring, instructing and teaching staff as needed.
Senior Community Care of Colorado (SCCCO) is a PACE Program serving older adults in Montrose and Delta Counties.
