Program Coordinator needed for part time position in Mental Health practice.

• Intake coordination and Trello card management including obtaining demographic information, consent forms and paperwork necessary for new patients, contacting family members, inputting into the EHR and computerized systems, and tracking each step of setting up a new referral in the program.
• Testing and review coordination includes scheduling initial evaluations and testing, tracking 3-month treatment reviews and annual testing due dates, and scheduling testing appointments. Also checking test diagnosis codes and formatting testing reports and forwarding them to communities and/or families on request.
• Patient relationships include speaking with new and existing patients on the phone to field questions, managing scheduling, and calling patients and families to check in, set up appointments, gather information, or explain the program.
• Maintaining community excel spreadsheets for each region with updated patient information.
• File management includes keeping all patient files up to date and audit ready.
• Sending birthday, sympathy, and holiday cards to patients and families.
  

• Contacting communities to explain the program and find the correct person to speak with in regard to the introduction of the memory training program on-site.
• Developing relationships with key contacts to coordinate services, alert to schedule changes, and assist with program as needed.
• Send reports, schedules, and weekly documentation as requested.
• Maintaining the community report with updated information.

• 
  Obtaining clinician’s schedules, creating and sending caseload lists, and contacting patients if a clinician is sick or out.
• Assisting with the EHR and clinical documentation as needed.
• Putting together starter toolkits and clinical resources.
