What are the responsibilities and job description for the Service Coordinator position at Mennonite Housing Rehabilitation Services, Inc.?
Job Overview:
Identify and facilitate the appropriate supportive or medical services necessary for HUD 202 residents to remain in and improve the quality of independent living. Services include activities of daily living (ADL’s) such as eating, dressing, bathing, grooming, transferring and home management.
Responsibilities:
- Provide general case management and referral services to all residents needing such assistance
- Establish linkages with all agencies and service providers in the community
- Set up directory of providers for use by both property staff and residents
- Refer and link residents to service providers in the community (i.e. case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy)
- Educate residents on service availability, application procedures, client rights, providing advocacy as appropriate
- Monitor the ongoing provision of services
- Help residents build informal support networks with other residents, family and friends
- Stay current on all changes to statutes and practices and obtain required HUD training and certification on an annual basis
Experience:
- Bachelor of Social Work or Gerontology degree preferred
- 2 years of experience working with senior population
- Excellent communication, writing, problem solving and organizational skills
- Attention to detail
- Ability to understand and maintain required confidentiality

Pay: From $16.00 per hour
