Job Summary
We are seeking a highly organized and compassionate individual to join our team as a Intervention Services Coordinator. Under the direction of the Behavioral Health Manager, the Intervention Services Coordinator is responsible for providing coordination over the operations, services, and personnel involved in outreach and court services.
Duties
- Work closely with individuals with developmental disabilities to assess their needs and develop individualized service plans
- Coordinate and monitor the delivery of services, ensuring that they are aligned with the goals and objectives outlined in the service plans
- Collaborate with a multidisciplinary team of professionals to ensure comprehensive care and support for individuals
- Provide ongoing guidance and support to individuals, their families, and caregivers
- Conduct regular assessments to evaluate progress and make necessary adjustments to service plans
- Maintain accurate and up-to-date documentation of all services provided
- Advocate for individuals' rights and access to community resources
- Assist in the development and implementation of programs aimed at enhancing the overall well-being of individuals
Requirements
- Bachelor's degree in social work or a related field
- Experience working with people with developmental disabilities
- Strong interpersonal skills with the ability to build rapport and establish positive relationships
- Excellent communication skills, both verbal and written
- Proven ability to effectively supervise and manage a team
- Experience in program development and management
- Knowledge of behavioral health principles and interventions
- Strong administrative skills, including budgeting and record keeping
