Description of Duties and Responsibilities
Under the supervision of the Director of Housing, the Case Manager is a non-clinical position that provides support services to tenants, helping them maintain stable housing and improve their quality of life. This role focuses on connecting tenants with resources, assisting them with daily living skills, and providing guidance to promote independence and housing stability. The Case Manager works collaboratively with community partners, service providers, and property management to ensure tenants receive the support they need.
Essential Functions and Responsibilities
- Build trusting relationships with tenants and provide individualized support.
- Assist tenants in accessing and navigating community resources, including employment, benefits, and financial assistance programs.
- Support tenants in developing life skills such as household management, self-care, and budgeting.
- Conduct regular check-ins with tenants to assess needs and progress towards wellness goals.
- Complete Coordinated Entry assessments as needed and maintain households in the Coordinated Entry System you are responsible for.
- Use evidence-based practices in service delivery such as Motivational Interviewing, Trauma Informed Care, Harm Reduction, and Housing First practices.
- Maintain accurate and timely documentation of computerized client records, mileage, and any other reports as directed.
- Collaborate with the on-site treatment team to coordinate clinical services.
- Advocate for tenants’ needs and ensure they have access to available programs and services.
- Assist tenants with conflict management and resolution. Assure tenants are adhering to rules-especially related to safety. Use appropriate de-escalation techniques when necessary.
- Work closely with property management to address housing stability concerns. Participate in team meetings and training sessions to enhance service delivery.
- Collaborate with other team members to keep the team informed of issues and progress.
- Other duties as assigned.
Qualifications:

: Practice harm reduction, motivational interviewing, trauma-informed care, confidentiality, and housing first. Human service delivery as it relates to persons experiencing homelessness, federal, state, and local regulations relating to fiscal and social services along with community resource programs.
: The Case Manager must be able to set and maintain professional boundaries. The ability to establish and maintain records, reports, and statistical data, effectively coordinate a variety of established policies and procedures, and use initiative and judgment in completing tasks and responsibilities. Involve individuals and community partners and the community at large to achieve objectives when appropriate.
: Associate’s or Bachelor’s degree in social work, human services, or a related field preferred or equivalent experience.
: Possession of a valid driver’s license, proof of auto liability insurance, and safe driving record.
Supervisory Relationships
This position reports to the Director of Housing.
Travel
Travel is primarily within the Vigo County area during operating hours, approximately 10% of the time.
Hours per Day or Week
This is a 1.0 FTE position with some benefits and flexibility to meet the needs of the organization.
Job Type: Full-time
Pay: $17.00 - $19.23 per hour
Expected hours: 40 per week
