What are the responsibilities and job description for the Case Manager I Intensive Outreach Navigation position at Mental Health Association of Oklahoma?
The Case Manager Intensive Outreach Navigation (ION) program will work in a mobile capacity, collaborating closely with the treatment teams of crisis and mental health centers. The Case Manager will collaborate closely with a Peer Recovery Support Specialist, under the guidance of the Supervisor of the Intensive Outreach Navigation program, as well as community partners and internal and external leasing partners. The Intensive Outreach Navigation team will outreach and serve individuals with Serious Mental Illness (SMI) who have been deemed as a frequent utilizer of the Oklahoma City area shelters, hospitals, and more specifically, the local area community mental health crisis centers such as the Oklahoma County Crisis Intervention Center, Griffin Memorial Mental Health Center, and the Oklahoma Crisis Recovery Unit. These individuals served via this program may also have had some involvement with the criminal justice system and/or experienced homelessness and need extensive support to live successfully in the community.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Build relationships with some of the most vulnerable, and thus difficult to assist, individuals within Oklahoma County (possibly housed, possibly homeless).
- Regularly collaborate with community partners involved in the healthcare systems and social support systems.
- Collect necessary data and information to show program outcomes and evaluation regarding the participants.
- Transport or assist in navigating public transportation for identified participants.
- Attending regularly scheduled meetings as determined by Supervisor and other partners, both internal and external.
- Collaborate with other referring agencies and personnel as needed.
- Assist participants in assessing, applying, and receiving appropriate social services, including but not limited to housing, OHFA vouchers, THA vouchers, TANF, Medicaid, Medicare, SSI, SSDI, etc. Perform other duties as assigned.
KNOWLEDGE, SKILLS & ABILITIES:
- This level of experience is usually obtained through a bachelor’s degree OR two years’ experience working with individuals who are homeless or in mental health/recovery services.
- At least 2 years of case management experience strongly preferred.
- The individual must possess outstanding communication skills and interpersonal comfort with people who may appear aggressive at times, people with histories of varying types of severe and chronic illness, personal hygiene issues, and untreated mental illness.
- Demonstrated experience in critical thinking and problem-solving skills will also be necessary.
WORK CONDITIONS & PHYSICAL DEMANDS:
- Work requires a flexible work schedule due to internal and external meetings. Occasional travel may be required; must have reliable transportation.
- Ability to effectively communicate orally and in writing.
- Physical ability to lift and carry 50 pounds occasionally.
- Visual and auditory ability to identify and respond to environmental and other hazards of the site and staff behavior.
- Physical mobility and endurance to perform tasks while standing/walking for long periods of time (60 minutes or more).
- Ability to safely and properly use office equipment.
- Potential exposure to stressful and/or physically challenging situations that could result in risk to one’s safety.
