Our organization is seeking a dedicated and compassionate Case Manager to support youth and young adults (ages 18-24) in our Greenfield YHDP programs. These programs provide essential housing and supportive services to individuals with disabilities who are striving to achieve stability and independence. The Case Manager will also provide in-person outreach services to our participants living in a variety of communities, including Greenfield, Athol, Holyoke, and surrounding areas. This is a temporary position due to the planned closure of the programs on 9/30/2025. However, we are looking to provide opportunities to transition into another program within Supportive Housing upon completion of the assignment.
Location: Chicopee, MA (with travel to Greenfield, Athol, Holyoke, and surrounding areas)
Position Type: Temporary, Full-Time (through 9/30/2025, with potential for transition to another program within Supportive Housing)
Responsibilities:
- Provide in-person outreach and case management to program participants across multiple communities.
- Assist individuals in securing and maintaining stable housing by helping with applications, landlord communication, and housing advocacy.
- Support participants in obtaining necessary documentation and benefits.
- Connect participants to mental health and substance use services as needed.
- Teach and model independent living skills, employment readiness, and educational goal setting.
- Conduct home visits and ensure housing meets safety and suitability standards.
- Maintain thorough and timely documentation, including assessments and progress notes.
- Provide transportation as necessary to support participants’ appointments and needs.
Requirements:
- Valid driver’s license, reliable vehicle, and sufficient automobile insurance.
- Ability to work independently while effectively collaborating with a team.
- Strong interpersonal skills, patience, and compassion when working with vulnerable populations.
- Good written, oral, and computer skills for documentation and communication.
- Flexibility and adaptability to meet participants' evolving needs.
- Ability to pass and maintain certifications in medication administration (MAP), CPR, and first aid.
Education and Experience:
- High school diploma or equivalent (GED) required.
- Experience working with individuals with disabilities is preferred.
- Experience in housing support services is a plus.
- Bilingual candidates and individuals with lived experience of homelessness are encouraged to apply.
Work Environment & Physical Demands:
- Travel is required to various locations to meet with program participants.
- Work is primarily conducted indoors but may require occasional exposure to outdoor elements.
- The role may require sitting, typing, and occasional lifting of up to 40 pounds.
