We're looking for dedicated CIS / Homeless Outreach Workers on Maui to make a real difference through hands-on engagement, resource connection, and advocacy.
As a CIS / Homeless Outreach Worker on Maui, you’ll be the vital link between individuals with serious mental illness and the services they need to stabilize, recover, and thrive. You'll work in the field, meeting people where they are, using empathy, persistence, and creative engagement strategies to connect individuals to care.
CIS / Homeless Outreach Worker Responsibilities:
- Outreach & Engagement: Provide flexible, field-based outreach to homeless adults experiencing mental illness. Build trust and rapport to connect individuals with services.
- Client Support & Coordination: Meet with assigned clients a minimum of four times per month. Use motivational interviewing and harm-reduction tools to support clients toward recovery goals.
- Service Linkage: Assist clients with obtaining IDs, registering for health plans, and accessing psychiatric/medication evaluations as needed.
- Data Collection & Documentation: Complete timely clinical documentation in accordance with MHK and Continuum of Care guidelines. Enter progress notes into NextStep within 24 hours of client contact.
- Housing Navigation: Participate in VI-SPDAT assessments and case conferencing to coordinate housing placements with community partners.
- Collaboration: Coordinate with case managers, the CIS Homeless Services Coordinator, and external providers to ensure wraparound care.
- Reporting: Submit monthly data reports, maintain confidentiality, and ensure accurate documentation in HMIS and other systems.
- Training & Compliance: Participate in supervision, in-service training, and maintain current CPR, First Aid, TB clearance, and background checks.
- Adaptability: Provide support outside traditional hours, including weekends or evenings as needed.
CIS / Homeless Outreach Worker Requirements:
- High school diploma required; bachelor’s degree in psychology, social work, or related field preferred.
- Three (3) years of experience in human services or mental health support roles.
- Understanding or willingness to develop skills in working with dual-diagnosis individuals (mental health & substance use).
- Strong interpersonal communication, crisis de-escalation, and problem-solving skills.
- Ability to work independently and as part of a collaborative team.
- Must pass a criminal background check and maintain current CPR, First Aid, TB clearance, and valid Hawaii driver’s license with clean abstract.
- Ability to lift up to 25 lbs and drive to outreach locations using personal vehicle.
