This is an exciting opportunity to be part of a first episode psychosis program offering community-based services to young adults in Boulder and Broomfield Counties. Being part of an interdisciplinary team means there is a ton of opportunities to learn new things, especially regarding how mental and physical health affect and support each other. This position typically operates on a standard five-day workweek during the initial training period. Upon successful completion of training, there is an option to transition to a four-day, 10-hour workweek (4/10 schedule), subject to operational needs and management approval.

Job Summary:

To provide supportive, community-based services to youth and young adults ages 15-29 who are experiencing their first episode of psychosis.  Will function on a wrap-around team to provide support around benefits, education, housing and family collaboration.  The case manager is responsible for providing direct support and skills training in collaboration with individuals, their families, and community supports. They will identify strengths and barriers within systems and utilize a strengths-based approach that focuses on early intervention strategies.
