What are the responsibilities and job description for the Front Desk and Community Outreach position at Mental Health Partners?
Position is full time. Schedule will vary between Monday through Saturday
Purpose: The Community Resource Liaison plays a role in reducing health disparities in underserved priority populations through identifying and understanding barriers to obtaining needed behavioral health services and seeking to mitigate those barriers. Position works at the front desk of Boulder Strong Resource Center and will also be responsible for community outreach
Job Summary: Community Resource Liaison will serve as a bridge between underserved communities and the behavioral healthcare system. The individuals in these roles are trusted members of their community and usually share ethnicity, language, socioeconomic status, and life experiences with the community they are serving. This enables the Community Resource Liaison/Front Desk to serve as a liaison/link/voice to partner agencies and the behavioral health care system as well as form relationships that facilitate entry into behavioral health care. Community Resource Liaisons are available to educate and facilitate enrollment to behavioral health services.
Essential Functions:
- Welcome and encourage guests to get connect with resources while working at the front desk at the Boulder Strong Resource Center
- Outreach to underserved priority populations within Boulder County and where these populations are most often found.
- Increase community member and partner agency education/knowledge about mental health facts, available behavioral health services, and avenues of access.
- Increase enrollment to behavioral health services (mental health and substance use treatment and to any service-system, not just MHP).
- Serve as cultural liaison by gathering information from persons in these underserved populations, collaborating with experts in the community, and sharing salient feedback within healthcare systems to positively influence quality, cultural responsiveness, and access to behavioral health services.
Specialized Duties:
- Actively connect and seek to build rapport with community members by being present, available and approachable in strategic locations
- Provide referrals and coordinate follow-through for community members accessing behavioral health services
- Work with community members to remove barriers, as possible, to behavioral health care
- Seek to get to know and understand community members, behavioral health care systems, and partner agency staff and systems, and professionally communicate lessons learned to key stakeholders with assistance of supervisor
- Assist and advocate for the community members to get into care when desired and help navigate the healthcare system
- Build trusting relationships within the community served and partners agencies to promote positive health and social outcomes
- Connect community members to appropriate care and/or services
- Provide culturally appropriate and relevant behavioral health information and education to interested community members and partner agencies as identified
- Attend and professionally represent MHP at key community events as determined by the Boulder Strong Resource Center supervisor
- Meet the defined team goals, activity metrics and Key Performance Indicators
- Promote, support and demonstrate MHP's mission, vision, and values through both behavior and job performance and adheres to policies, procedures, Compliance Plan and Standards of Conduct on a day-to-day basis
- Effectively interacts and communicates with other MHP staff/clients/customers/partners/etc.
- Regular and consistent attendance is required to perform other essential functions of the job
- Other duties as assigned
